Oftentimes a patient is struggling with more than one chronic disease, so we look for the treatment options and lifestyle modifications that will have the greatest results.
Prevention is also an important part of disease management. We will identify your risk factors for diseases that could arise later on and then make recommendations for the changes you can make now to preserve your health in the future.
We can work in partnership with your specialists by providing more frequent care and the encouragement to follow their treatment plans. Sometimes there may be a wait to see a specialist, so you can come to us in the meantime. If we discover that there is an urgent need for their care, we can help you to coordinate an appointment with them at the soonest date.
